Step 1

Complete online application ($30 application fee)

Step 2

Mail Transcripts and Test Scores

Send your application materials to this mailing address:

  •     Please only send one copy of your official transcript.
  •     GRE scores sent directly to FL POLY, via our institutional code of #5233.
  •     If required, TOEFL scores sent directly to FL POLY, code # 0071.We cannot accept photocopies in lieu of official results from the testing service(s). All photocopies of scores will be discarded and the requirement will remain unmet.

Step 3

Submit 2 letters of recommendation (professional or from a previous professor)

Step 4

Submit Personal Statement Essay (500 words or less)

Student Responsibility: The student is responsible for completing the university’s application by the stated deadline. The student is also responsible for submitting all documents to the FL POLY by the stated deadlines.
